This notice covers aixworksitehealth.com, run by AIX Group, LLC ("we"). It explains what we collect through this site and what we do with it.
What we collect
- Employer requests: your name, company, work email, phone, worksite ZIP codes, the approximate number of employees at each site, and the services you're interested in.
- Clinic enquiries (call requests, pricing requests and the waitlist): your name, clinic name, email, phone where asked, base city or area, service line and the territory you're interested in.
- Basic technical data: our hosting provider keeps standard request logs (such as IP address and browser type) to run and protect the site.
We don't ask for, and you shouldn't send, health information about anyone. Care is arranged between employees and the independent clinic that provides it; this site is not part of that.
What we do with it
- We pass an employer request to the independent clinic licensed for that area and service, or keep it on a waitlist until one is.
- We use clinic enquiries to arrange calls, share territory pricing and tell you when a territory opens.
- We don't sell your information, and we don't use advertising trackers on this site.
Who handles it
We use service providers to host the site and to manage contacts and email on our behalf. They may only use the information to provide that service to us.
How long we keep it
We keep a request while we're working on it and for up to 24 months after our last contact with you if it doesn't lead to a working relationship, then delete it. Records connected to a signed agreement are kept for as long as the agreement and the law require.
Your choices
You can ask us to see, correct or delete what we hold about you, or to stop contacting you, through the contact page. Every email we send has an unsubscribe link.
Changes
If we change this notice, we'll update the date at the top of this page.
